Why did my I-Share request fail?

I-Share requests can fail for a number of reasons. The book could have been checked out (or otherwise became un-borrowable) between the time you placed your request and when the lending institution picked it from the shelf, or it could have been mis-shelved or lost without the library noticing and marking it as unavailable in their catalog.
